PACE Southeast Michigan is seeking a Master’s Social Worker (MSW) to autonomously assess needs, coordinate, and deliver social services for participants and caregivers. The MSW serves on the Interdisciplinary Team (IDT) and participates in assessment, intervention, management, and review of social service needs.

The role emphasizes bio-psychosocial assessment, CBT-informed interventions, psychoeducation, and comprehensive care planning and case management to support participants and families.

Competenze

• Master’s Social Worker degree from an accredited school.
• Licensed to practice in Michigan (Limited License considered depending on supervised hours).
• At least 1 year of experience as a clinical social worker, preferably in geriatrics.

Mansioni

• Conducts comprehensive bio-psychosocial assessments at enrollment and at 180-day intervals.
• Develops and implements individualized treatment plans including counseling, education, and care coordination.
• Directs case management involving housing, finances, safety, and discharge planning.
