PACE Southeast Michigan in Pontiac seeks a Master’s Social Worker to join its Interdisciplinary Team (IDT). The MSW independently assesses needs, coordinates social services, and delivers care for participants and families using bio-psychosocial approaches.

The role involves developing individual treatment plans, counseling, education, and care coordination with the care team. Documentation in the EHR and ongoing progress reviews are required.

Qualifikationen

• Master’s Social Worker from an accredited school of social work.
• Licensed to practice in the State of Michigan.
• One year experience as a clinical social worker, preferably in geriatrics.
• Knowledge of case management, patient-centered approaches, and CPD.
• Proficient with electronic health records and documentation.

Aufgaben

• Assess needs, coordinate, and deliver social services for participants and families.
• Develop and implement treatment plans including counseling and education.
• Coordinate services with the care team and perform risk assessment/crisis intervention.
• Assist in discharge planning and education of participants and caregivers.
• Advocate for participants and maintain timely communication with the IDT.
