Job summary

Baptist Health Floyd in Southern Indiana seeks a Lead Social Worker for a full-time, Monday–Friday role. You will assist the Director of Case Management in mentoring staff and overseeing daily Social Work operations.

The role includes discharge planning, case management, and psychosocial support for patients and families during hospitalization, with emphasis on education and continuity of care after discharge.

Qualifications

• Must have MSW from CSWE-accredited program.
• Current Kentucky CSW license or Indiana LSW license required.
• Minimum three years of social work experience; CCM preferred.

Responsibilities

• Assist Director of Case Management with staff mentorship and day-to-day operations.
• Conduct psychosocial assessments and provide discharge planning.
• Deliver psychosocial support to patients and families during hospitalization.
• Advise on non-medical barriers to care and continuity of post-discharge options.
