Baptist Health Floyd in New Albany, IN is seeking a Lead Social Worker for a full-time, Monday–Friday, 8:00a–4:30p schedule. The role supports the Director of Case Management through staff development, discharge planning, and psychosocial support within a hospital setting in Southern Indiana/Louisville area.

资格要点 include an MSW and active CSW/LSW license with CCM preferred; minimum 3 years’ social work experience desired. EOE.

Qualifications

• MSW degree from an accredited program.
• Current Kentucky CSW or Indiana LSW license required.
• CCM certification preferred.
• Minimum 3 years of social work experience preferred.

Responsabilités

• Mentor staff and support day-to-day operations of the Social Work team.
• Perform psychosocial assessments and interventions to support patients and families.
• Provide discharge planning and case management to facilitate timely and safe discharge.
• Advise on non-medical barriers to care and coordinate continuity of care after discharge.
