Job summary

Baptist Health Floyd in Southern Indiana seeks a Lead Social Worker for a full-time, Monday–Friday role. You will assist the Director of Case Management in mentoring staff and overseeing daily Social Work operations.

The role includes discharge planning, case management, and psychosocial support for patients and families during hospitalization, with emphasis on education and continuity of care after discharge.

Qualifications

• Must have MSW from CSWE-accredited program.
• Current Kentucky CSW license or Indiana LSW license required.
• Minimum three years of social work experience; CCM preferred.

Responsibilities

Summary m-f 8a-4:30p Job Description: Job Description Seeking Lead Social Worker! Full-time, Monday-Friday, 8:00a-4:30p Baptist Health Floyd, Case Management Southern Indiana/Louisville, Kentucky area The Lead Social Worker is a non-clinical social work position, which assists the Director of Case Management in the mentorship and development of staff as well as the management of day-to-day operations of the Social Work team. In addition, the Lead Social Worker is responsible for discharge planning, case management, and psychosocial support of patients and their families within the hospital setting. This involves completing psychosocial assessments and interventions to assist patients in coping with their hospitalization, illness, diagnosis, and/or life situation and provide support and education to families on how to support the patient after hospitalization. The Social Worker also provides advice or guidance for any other non-medical problem or barriers to care patients may have that may hinder their rate of recovery or delay discharge and serves as a resource for continuity of care options after discharge.

Qualifications

• Current Master of Social Work degree from a school of social work accredited by the Council on Social Work Education required.
• Current Kentucky license as a Certified Social Worker (CSW) required or Current Indiana license as a Licensed Social Worker (LSW) required based on state of practice.
• Minimum of three (3) years of social work experience preferred.
• Case Management Certification (CCM) preferred.
