Job summary

Baptist Health Floyd in New Albany, IN is seeking a Lead Social Worker to join the Case Management team. The role is full-time, days, Monday through Friday, 8:00 am to 4:30 pm, and reports to the Director of Case Management.

The Lead Social Worker will mentor staff, oversee daily operations, perform psychosocial assessments, discharge planning, and provide support to patients and families to facilitate smooth transitions from hospital care.

Qualifications

• MSW degree from a school of social work accredited by CSWE.
• Current license as CSW (KY) or LSW (IN) depending on state of practice.

Responsibilities

• Discharge planning and case management for hospitalized patients.
• Provide psychosocial support to patients and families.
• Perform psychosocial assessments and interventions to aid coping with hospitalization.
• Offer guidance for non-medical barriers to care and post-discharge options.

In addition, the Lead Social Worker is responsible for discharge planning, case management, and psychosocial support of patients and their families within the hospital setting. This involves completing psychosocial assessments and interventions to assist patients in coping with their hospitalization, illness, diagnosis, and/or life situation and provide support and education to families on how to support the patient after hospitalization.
