Job summary

Baptist Health Floyd in New Albany, IN is seeking a Medical Social Worker (MSW) for PRN, hospital-based care coordination and psychosocial support. The role focuses on discharge planning, family education, and ensuring smooth transition after hospitalization.

The MSW will assess psychosocial needs, provide interventions, and collaborate with a multidisciplinary team to support patients and families across Southern Indiana and the Louisville area.

Qualifications

• Masters of Social Work from a CSWE-accredited program.

Summary Job Description: Seeking Medical Social Worker! PRN (work as needed), shifts and days vary Baptist Health Floyd, Care Coordination Southern Indiana/Louisville, Kentucky area The Social Worker (MSW) is a non-clinical social work position responsible for discharge planning, case management, and psychosocial support of patients and their families within the hospital setting.

This involves completing psychosocial assessments and interventions to assist patients in coping with their hospitalization, illness, diagnosis, and/or life situation and provide support and education to families on how to support the patient after hospitalization.

The Social Worker also provides advice or guidance for any other non-medical problem or barriers to care patients may have that may hinder their rate of recovery or delay discharge and serves as a resource for continuity of care options after discharge.

Qualifications
