Job summary

Complete Care in New Jersey is seeking a Director of Social Services (LSW/CSW) to lead the Social Services Department and enhance residents' quality of life through psychosocial support, discharge planning, and family guidance.

The role requires current LSW or CSW, supervisory experience, strong communication and documentation skills, and the ability to collaborate with the interdisciplinary clinical team in a warm, respectful workplace.

Qualifications

• Current LSW or CSW license required.
• Long-term care experience preferred.
• Supervisory or management experience preferred.
• AHA CPR certification recommended.
• Strong communication, organizational, and documentation skills.
• Computer proficiency required.
• Ability to work independently and as part of an interdisciplinary team.

Responsibilities

• Lead and oversee the Social Services Department at Complete Care
• Develop and implement social service programs, policies, and goals
• Advocate for residents' psychosocial, emotional, and behavioral health needs
• Participate in interdisciplinary care plan meetings and collaborate with the clinical team
• Maintain accurate, timely, and compliant documentation
• Educate residents and families regarding Advance Directives and available resources
• Coordinate admissions, transitions, and discharge planning to ensure positive outcomes
• Provide guidance and support to social services staff
• Assist residents and families with community resources and support services
• Perform additional duties as assigned by the Administrator
