Job summary

Complete Care - New Jersey & PA is seeking a Director of Social Services (LSW/CSW) to lead the Social Services Department, address residents' psychosocial needs, and support families throughout the healthcare journey.

You will develop programs, coordinate admissions and discharge planning, and collaborate with the clinical team to ensure compliant documentation and positive resident outcomes. Strong communication and leadership are essential.

Qualifications

• Current LSW or CSW required.
• Supervisory/management experience preferred.
• AHA BLS CPR certification recommended.
• Strong communication, organizational, and documentation skills.
• Ability to work independently and as part of an interdisciplinary team.

Responsibilities

• Lead and oversee the Social Services Department.
• Develop and implement social service programs, policies, and goals.
• Advocate for residents' psychosocial, emotional, and behavioral health needs.
• Participate in interdisciplinary care plan meetings and collaborate with the clinical team.
• Maintain accurate, timely, and compliant documentation.
• Educate residents and families regarding Advance Directives and resources.
• Coordinate admissions, transitions, and discharge planning for positive outcomes.
• Provide guidance to social services staff.
• Assist residents and families with community resources and support services.

Director Of Social Services (LSW/CSW) Qualifications

• Current LSW or CSW required
• Long-term care experience preferred, but not required
• Prior supervisory or management experience preferred
• Current AHA BLS CPR Certification recommended
• Strong communication, organizational, and documentation skills
