Job summary

Complete Care in New Jersey is seeking a Director of Social Services. The role offers a full-time day shift with a sign-on bonus and a comprehensive benefits package.

The ideal candidate will hold an LSW or CSW and bring prior long-term care experience to lead the Social Services team. You will supervise social work staff, ensure compliant documentation, and participate in care planning to support residents and families.

Qualifications

• LSW or CSW is required for this role.
• Long-term care experience is preferred.
• Experience managing social work staff is preferred.
• Strong documentation and record-keeping skills are essential.

Responsibilities

• Directs and plans the Social Services department policies and goals.
• Maintains accurate, thorough, and timely documentation.
• Participates in interdisciplinary care plan meetings to address residents’ social and emotional needs.
• Educates residents and families on advanced directives and care options.
• Manages smooth admissions and discharges for residents and families.
• Collaborates with the administrator and other staff to ensure quality care.

Director of Social Services Schedule & Benefits

• Schedule: Full-time, day shift position, including Monthly Manager on Duty weekend rotation

Director of Social Services Qualifications & Responsibilities

• Must have LSW or CSW


• Previous long term care experience is preferred


• Prior experience managing social work staff is preferred


• Directs, establishes & plans the overall policies/goals for the Social Services Dept


• Maintain accurate, thorough, and timely documentation


• Participates in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident


• A wide degree of creativity and latitude is expected


• Review Advanced Directives and provide education to residents and family members as needed


• Help provide a smooth admission and discharge process for residents and family members


• Ability to work independently or part of an interprofessional team


• Computer knowledge and experience using Point Click Care


• Other duties as assigned by Administrator
