Job summary

Complete Care - Connecticut is hiring an LSW Licensed Social Worker Director of Social Services to lead the social work team in our Connecticut facility. The role focuses on supervision, policy development, and ensuring regulatory compliance while supporting residents and families.

We seek a CT-licensed LSW with long-term care experience, strong leadership, and a commitment to quality care. The position offers full-time hours, day shift, comprehensive benefits, and opportunities for advancement.

Qualifications

• Active Connecticut LSW license.
• Leadership experience in long-term care.
• Ability to pass background check and screenings.

Responsibilities

• Supervises staff, hires, trains, and schedules social work staff and interns.
• Establishes departmental policies, procedures, and quality improvement standards & oversees the social services department budget and resource allocation.
• Enforces Regulations: Ensures the facility complies with federal (CMS), state, and local social service mandates & Serves as the primary department contact during state health inspections and surveys
• Maintains Documentation: Maintains & audits medical charts to ensure psychosocial documentation meets strict legal standards.
• Collaboration: Participates in care planning and other meetings, including Quality Assurance and Performance Improvement (QAPI).
• Care & Mediation: Steps in resolving severe, complex family grievances or resident crises. Provides supportive care as needed to supplement social work needs
• Community Liaison: Builds relationships with local hospitals, mental health organizations, and community resources to streamline admissions and discharges.

Does This Sound Like You?

• Licensed to work within the State of Connecticut as a LSW Licensed Social Worker
• Able to pass background check, drug screen, professional references check & physical evaluation
• High level understanding of leadership, customer service and clinical competency
• Leadership experience in a long-term care setting
• Prior experience as a Director of Social Services/Social Work Director
