Job summary

Complete Care is seeking a Director of Social Services (LSW/CSW) to lead the Social Services Department and enhance residents' quality of life. You will address emotional, psychosocial, and discharge planning needs while supporting families throughout their healthcare journey.

The role offers a full-time day shift with rotating weekend coverage and a comprehensive benefits package. Strong leadership and collaboration with the interdisciplinary team are essential to success.

Qualifications

• Current LSW or CSW required.
• Long-term care experience preferred but not required.
• Prior supervisory or management experience preferred.
• Current AHA BLS CPR Certification recommended.
• Strong communication, organizational, and documentation skills.
• Computer proficiency required.
• Ability to work independently and as part of an interdisciplinary team.

Responsibilities

• Lead and oversee the Social Services Department at Complete Care.
• Develop and implement social service programs, policies, and goals.
• Advocate for residents' psychosocial, emotional, and behavioral health needs.
• Participate in interdisciplinary care plan meetings and collaborate with the clinical team.
• Maintain accurate, timely, and compliant documentation.
• Educate residents and families regarding Advance Directives and available resources.
• Coordinate admissions, transitions, and discharge planning to ensure positive outcomes.
• Provide guidance and support to social services staff.
• Assist residents and families with community resources and support services.
• Perform additional duties as assigned by the Administrator.
