Job summary

Complete Care at Brick is seeking a Director of Social Services (LSW/CSW) to lead the Social Services Department and address residents' psychosocial needs and discharge planning. In this role, you will collaborate with the clinical team, educate residents and families about Advance Directives, coordinate admissions and transitions, and ensure compliant documentation.

• Current LSW or CSW required.
• Supervisory or management experience preferred.
• Strong communication, organizational, and documentation skills.
• Computer proficiency required.
• Ability to work independently and as part of an interdisciplinary team.

Responsibilities

• Lead and oversee the Social Services Department at Complete Care.
• Develop and implement social service programs, policies, and goals.
• Advocate for residents' psychosocial, emotional, and behavioral health needs.
• Participate in interdisciplinary care plan meetings and collaborate with the clinical team.
• Maintain accurate, timely, and compliant documentation.
• Educate residents and families regarding Advance Directives and available resources.
• Coordinate admissions, transitions, and discharge planning to ensure positive outcomes.
• Provide guidance and support to social services staff.
• Assist residents and families with community resources and support services.
• Perform additional duties as assigned by the Administrator.

• Current LSW or CSW required
• Long-term care experience preferred, but not required
• Prior supervisory or management experience preferred
• Current AHA BLS CPR Certification recommended
