Job summary

Tri-Counties Regional Center is seeking a Lead Service Coordinator to support the Services and Supports Team Manager in person-centered planning and advocacy. You will mentor staff, coordinate IPP/IFSP processes, and ensure compliant service delivery while maintaining a reduced caseload.

Qualifications

• Master’s or Bachelor’s in human services with 2 years relevant experience.
• 2 years Regional Center experience preferred.
• Reliable transportation and ability to travel.
• Bilingual Spanish/English preferred.

Responsibilities

• Provide training and mentoring to new employees and assist in coordinating ongoing team training.
• Provide person-centered training and mentoring and participate in PCT meetings.
• Review case records for Medicaid Waiver compliance and reenrollment.
• Review and approve IPP/IFSPs and related authorizations.
• Help organize coverage when team members are out for extended periods.
• Support individuals with ongoing service coordination and family involvement.
• Attend Interdisciplinary Team meetings and community meetings as needed.
• Coordinate and secure funded services, including behavior services and independent living supports.
