Job summary

Tri-Counties Regional Center is seeking a qualified professional to provide person-centered consultation, advocacy and service coordination for people with developmental disabilities. The role collaborates across specialists to ensure effective planning and access to resources.

Responsibilities include IPP development, reassessments and quarterly reviews, with a focus on empowering individuals and families to engage with TCRC and community partners.

Qualifications

• Master’s or Bachelor’s degree in human services field with 2 years of applicable experience in mental health or social work.
• 2 years experience case management within a related field.
• 1 year experience with individuals with developmental disabilities.
• Experience with Person Centered Thinking (PCT) and Person-Centered Planning (PCP).
• Bilingual and bicultural English/Spanish required.

Responsibilities

• Manage a 1:40 caseload ratio with POS considerations.
• Develop IPP, progress reports, annual reviews, and quarterly in-person reviews.
• Coordinate with PCP Specialist and other teams to identify resources.
• Advocate for individuals and families within public systems.
• Participate in outreach, community meetings, and inter-agency committees.
