Job summary

The Oaks at Byron Center, part of Trilogy Health Services, seeks a Registered Nurse (RN) to provide direct, high-quality clinical care and to serve as a Team Leader for patient care staff. You will oversee medications, care planning, and family teaching while ensuring adherence to policies and procedures.

In this role, you’ll guide a team on a daily basis, observe residents, and respond to changes in condition with sound clinical judgment.

Qualifications

• Must have and maintain a current, valid state RN license.
• Current, valid CPR certification required.

Responsibilities

• Lead a team of direct care providers to ensure medications, treatments, documentation, family teaching, care planning and patient care conferencing in line with campus policies.
• Utilize the nursing process to deliver patient care and ensure continuity from admission through discharge and return to family.
• Visit residents daily to observe status and notify the charge nurse/physician when there is a change in condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Perform administrative duties such as completing forms, reports, evaluations, and charting as needed.
