Job summary

Devoted Health is seeking a Manager of Social Work Case Management to build, lead, and inspire a remote team of Social Worker Case Managers and behavioral health care coordinators within Transitions of Care (TOC). You will collaborate with cross-functional leaders to drive outcomes and ensure continuity of care from admission through post-discharge.

The role emphasizes building workflows, coaching staff, and managing a fully remote schedule (M–F, 8:30am–5:00pm in CST/MST/PST).

Qualifications

• Active social work license (LMSW or higher) required.
• 3+ years in case management.
• 3+ years in people management.
• Experience with interdisciplinary care management teams.
• Ability to manage a fully remote schedule (M–F 8:30am–5:00pm CST, MST, or PST).

Responsibilities

• Build and develop a high-performing team of Social Worker Case Managers and care coordinators.
• Track staff performance through competency check-offs, audits, shadowing, and 1:1s.
• Create workflows, assessments, and plans of care for efficiency and better patient outcomes.
• Support the team on escalated patient needs.
• Lead onboarding for new teammates and ongoing staff education.
• Partner with cross-functional teams to standardize workflows across programs.
• Meet regularly with TOC leadership to review performance and improvements.
• Spend 30–40% of time on direct patient outreach and case work.
