Job summary

Devoted Health is seeking a Manager of Social Work Case Management to build and lead a remote team of case managers and behavioral health coordinators within the TOC program. You will collaborate with cross-functional teams and oversee workflows that improve patient outcomes and continuity of care.

The role requires an active LMSW or higher license, 3+ years in case management and people leadership, and the ability to manage a fully remote schedule across CST/MST/PST.

Qualifications

• Active LMSW license or higher.
• 3+ years of experience in case management.
• 3+ years of experience in people management.
• Experience working with interdisciplinary care management teams.
• Ability to independently manage a fully remote schedule, M–F, 8:30am–5:00pm in CST/MST/PST.

Responsibilities

• Build and develop a high-performing team of Social Worker Case Managers and behavioral health care coordinators.
• Track staff performance through competency check-offs, audits, shadowing, and 1:1s, and coach where needed.
• Create workflows, assessments, and plans of care that improve team efficiency and patient outcomes.
• Support the team on escalated patient needs.
• Lead onboarding for new teammates and ongoing staff education.
• Partner with cross-functional teams to drive execution and standardize workflows.
• Meet with Senior Manager of RN and SW TOC functions to review performance and improvements.
• Spend 30–40% of time on direct patient outreach and casework.
