Job summary

Devoted Health is seeking a Manager of Social Work Case Management to lead TOC teams that support patients from admission through 30 days post-discharge. You will build a high-performing group of Social Worker Case Managers and behavioral health care coordinators, partnering with cross-functional teams to drive outcomes and efficient workflows.

The role requires an active LMSW or higher license, 3+ years in case management and people leadership, and the ability to manage a fully remote schedule

Qualifications

• Active LMSW license (or higher).
• 3+ years of case management experience.
• 3+ years of people management experience, building high-performing teams.
• Experience with interdisciplinary care management teams preferred.
• Ability to independently manage a fully remote schedule (M–F, 8:30am–5:00pm CST/MST/PST).

Responsibilities

• Build and develop a high-performing team of Social Worker Case Managers and behavioral health care coordinators.
• Track staff performance through competency checks, audits, shadowing, and coaching.
• Create workflows, assessments, and plans of care to improve team efficiency and patient outcomes.
• Support the team on escalated patient needs.
• Lead onboarding for new teammates and ongoing staff education.
• Partner with cross-functional teams to standardize workflows across programs.
• Meet with Senior Manager of RN and SW TOC functions to review performance and identify improvements.
• Spend 30–40% of time on direct patient outreach and case work.
