Colwyn Group is seeking a Primary Care Physician – Pod Leader in the San Jose/North Hollywood area to deliver comprehensive, community-based care for geriatric populations. The role emphasizes interdisciplinary collaboration within a small patient panel and leadership of IDT members.

The ideal candidate has 2–5 years in primary care, ACGME residency, and is licensed to practice in California. The position offers relocation assistance and a structured, weekday schedule with on-call coverage as

Qualifications

• ACGME-accredited residency completed.
• MD/DO degree required.
• Active, unrestricted medical license in the applicable state.
• DEA registration and CDS certification.
• Board certification preferred.
• Eligible to enroll in Medicare and Medi‑Cal.
• CPR and First Aid certification within 180 days of hire.

Responsibilities

• Conduct comprehensive in-person patient assessments.
• Lead and collaborate with the interdisciplinary team (IDT).
• Develop individualized care plans for patients.
• Coordinate placement and discharge of care.
• Provide primary care without hospital rounds.
• See ~8 patients per day.
• Participate in rotating on-call schedule by phone.
• Schedule Monday–Friday 8:00 AM–5:00 PM.
• Weekends generally off.
• Rotating phone-based on-call coverage a few nights or weekends per month
