Job summary

PACS is seeking a licensed Physical Therapist for our post-acute care facility. The PT will evaluate patients within 24 hours of referral, develop treatment plans, supervise PT assistants, and participate in discharge planning.

The role requires licensure in the state, a bachelor’s or higher in physical therapy, and proficiency with Casamba and Point Click Care. We offer comprehensive benefits and opportunities for professional growth.

Qualifications

• Bachelor’s degree in Physical Therapy, prefer graduates of Masters or Doctorate Program in Physical Therapy.
• Proficient in computer skills such as inputting data into programs such as Casamba and Point Click Care.

Responsibilities

• Evaluate patients within 24 hours of referral; develop treatment plans and obtain approval from referring physician.
• Treat patients per the physician treatment plan and supervise PT assistants.
• Record evaluations, treatment notes, progress notes, and discharge planning documentation.
• Instruct families and nursing staff in maintenance and caregiver training for discharge.
• Participate in in-services, care conferences, and rehabilitation conferences as needed.
