Job summary

PACS Group is seeking a licensed Physical Therapist to evaluate patients within 24 hours of physician referral and to develop effective treatment plans. You will supervise PT assistants and students, coordinate with the health team, and ensure documentation meets state and payer requirements.

You will also participate in discharge planning, family education, and equipment recommendations to support patient independence, with a focus on quality care and compliance with regulations.

Qualifications

• Ability to read technical procedures and policy manuals.
• Licensed physical therapist in the state; maintain CE requirements.
• Proficient computer skills for data entry in clinical software.

Responsibilities

• Evaluate patients within 24 hours of referral or 1 business day.
• Develop and obtain approval for treatment plans from referring physician.
• Supervise PT assistants and students in direct patient care.
• Document evaluations, progress notes, and discharge planning.
• Instruct families and nursing staff for discharge preparation.
