Qualifications

• Bachelor's degree in Physical Therapy; Masters or Doctorate preferred.
• Proficient in Casamba and Point Click Care.
• Licensed Physical Therapist in the state; maintain CE requirements.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of physician referral.
• Develop treatment plan and obtain approval from referring physician.
• Treat patients per physician treatment plan.
• Supervise Physical Therapy Assistants, aides and students.
• Record evaluations and treatment notes per state practice act and payer requirements.
• Instruct families or nursing staff in maintenance and caregiver training for discharge.
• Participate in discharge planning and patient care conferences.
• Recommend and facilitate ordering of durable medical equipment to aid mobility.
