Job summary

MONROEVILLE is seeking a licensed Physical Therapist to evaluate patients within 24 hours or one business day of physician referral and to develop effective treatment plans in coordination with physicians.

You will supervise PT assistants and students, document treatments, and assist with discharge planning and family training in mobility and home programs. Continuous education and collaboration with the health team are required.

Qualifications

• Bachelor's degree in Physical Therapy; prefer graduates of Masters or Doctorate Program.
• Licensed Physical Therapist in the state; keep CE requirements up to date.
• Proficient in Casamba and Point Click Care data entry and documentation.

Responsibilities

• Evaluate patients within 24 hours or 1 business day of referral.
• Develop and obtain approval for treatment plans from referring physician.
• Supervise PT assistants, aides and students in patient care.
• Document evaluations and treatments per PT Board and payer requirements.
• Instruct families and nursing staff for discharge and home programs.
• Participate in discharge planning, care conferences and staff training.
