Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and aides in resident care.
• Communicate progress with physicians, nursing staff, residents, and families.
• Maintain timely, accurate documentation per facility and regulatory requirements.
