Job summary

Heritage Place Care & Rehabilitation is seeking a Physical Therapy Assistant to join our in-house team. You will provide compassionate PTA services in skilled nursing, assisted living, and outpatient settings, under the supervision of a Licensed Physical Therapist.

Primary duties include implementing the plan of care, communicating patient progress to the PT, and ensuring compliance with practice acts and facility policies. Pay is based on experience.

Qualifications

• Active state licensure or certification is required.

Responsibilities

• Treat patients according to the treatment plan of care developed by and under the supervision of the Licensed Physical Therapist.
• Communicate with the Physical Therapist any pertinent information concerning patient progress, concerns, and plans.
• Be proficient in therapeutics needs to treat patients based on their presentation and instructions given by the Physical Therapist.
• Participate in patient Plan of care meetings if asked by the Director of Rehab.
• Record treatment notes, progress notes, provide objective input for discharge summaries and complete daily billing of services per policy and procedures.
• Adherence to the Required practice acts, code of conduct, ethics during professional practice as an Physical Therapy Assistant.
• Adherence to Company requires policy, procedures and code of conduct.
