Job summary

Qualifications

• Licensed PT in the state of practice.
• Graduate degree in physical therapy preferred.
• Proficient with documentation systems (Net Health, PCC).

Responsibilities

• Evaluate residents to determine PT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility.
• Supervise PT assistants and coordinate with the care team.
• Maintain timely, accurate documentation for compliance and payers.
• Educate residents and families on home programs and safety.
