Job summary

Sunnyside Healthcare Center is seeking a Physical Therapist to evaluate residents and determine therapy needs according to physician orders and facility policies. The role encompasses developing customized treatment plans to improve mobility, strength, balance, endurance, and functional independence.

The PT will supervise PT assistants and other staff in resident care, work with physicians and nursing teams, and ensure accurate, compliant documentation while promoting safety and fall prevention.

Qualifications

• Diagnose and evaluate residents to determine physical therapy needs per physician orders and facility policies.
• Develop, implement, and update individualized treatment plans to improve mobility, strength, balance and independence.
• Provide training and supervision of PT assistants and other staff in resident care and rehabilitation activities.

Responsibilities

• Supervise PT assistants (PTA), aides, and students.
• Communicate progress, goals, and discharge planning with physicians, nursing staff, and families.
• Complete timely documentation in accordance with facility and payer requirements.
• Maintain infection control, safety, and equipment maintenance protocols.
• Educate residents and caregivers on goals, exercises, safety and fall prevention.
• Coordinate ordering of durable medical equipment to support mobility and independence.
