Job summary

SUNNYSIDE is seeking a licensed Physical Therapist to evaluate residents, determine therapy needs, and design individualized treatment plans to improve mobility and independence in line with physician orders and facility policies.

You will supervise Physical Therapy Assistants, educate residents and families, coordinate discharge planning, document thoroughly, and participate in care conferences and training sessions to support quality rehab outcomes.

Qualifications

• Evaluate residents to determine physical therapy needs according to physician orders and facility policies.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Assist nursing with training of Restorative Aides.
• Supervise PT Assistants in resident care per state practice act.
• Communicate with physicians, nursing staff, and families regarding progress and discharge planning.
• Participate in care conferences and rehabilitation meetings as needed.
• Provide in-services and ensure timely, accurate documentation.
• Maintain a safe environment with infection control and equipment maintenance.

Responsibilities

• Supervise Physical Therapy Assistants in direct resident care and related activities.
• Educate residents, families, and caregivers on treatment goals and home exercise programs.
• Contribute to quality assurance and rehabilitation program development.
• Report equipment problems and ensure a safe treatment environment.
• Relate positively with residents, families, staff, and colleagues.
