Job summary

Sunnyside SNF Healthcare, LLC is seeking a Physical Therapist to evaluate residents, develop treatment plans, and supervise PT Assistants in direct resident care. You will collaborate with physicians and nursing staff, participate in care conferences, educate residents and families, and maintain compliant documentation.

Qualifications

• Experience supervising PT assistants
• Ability to educate residents and families on goals and home exercise programs
• Commitment to confidentiality of residents' information
• Adherence to infection control protocols and safety procedures

Responsibilities

• Evaluate residents to determine physical therapy needs per physician orders
• Develop, implement, and update individualized treatment plans to improve mobility and independence
• Assist nursing department with training of Restorative Aides
• Supervise Physical Therapy Assistants in resident care
• Communicate regularly with physicians, nursing staff, residents, and families regarding progress and discharge planning
• Participate in Resident Care conferences, Utilization Review meetings, and Rehabilitation meetings
• Provide in-services on training programs for staff
• Complete timely, accurate documentation in accordance with policies
• Maintain a safe treatment environment by following infection control and safety protocols
• Educate residents and families on goals, home exercise programs, and safety techniques
• Recommend and facilitate ordering of needed durable medical equipment
• Maintain confidentiality of residents' information and company data
• Report issues with department equipment to keep it in good working order
• Contribute to quality assurance and rehab program development
• Relate positively with residents, families, staff, and colleagues
