Job summary

Stonerise in Ohio is seeking a Home Health Physical Therapist (PT) to provide PT services per patient plan of care, evaluate strength, mobility, gait, and ROM, and supervise PTAs. The role emphasizes leadership, patient-family education, discharge planning, and coordination with the home health team.

Requires a PT degree, state licensure, at least two years of PT experience (home health preferred), and a valid driver's license.

Qualifications

• Bachelor’s degree or higher in physical therapy from an accredited program.
• Licensed to practice as a Physical Therapist in the state.
• At least two years of PT experience; home health experience preferred.
• Valid Driver’s License and a vehicle in good working order.

Responsibilities

• Provide physical therapy services per patient plan of care, assessing strength, mobility, gait, ROM.
• Instruct patients and families/caregivers in therapy phases and home exercises.
• Train patients on use of prosthetic devices.
• Identify needs for other home health services and refer as appropriate.
• Prepare and submit clinical/progress summaries aligned with patient goals.
• Participate in discharge planning and 30-day reassessment revisions.
• Provide PT consultation to home families/caregivers and supervise PT assistants.

As a Stonerise Home Health Physical Therapist (PT), you will be responsible for providing physical therapy services and are responsible for the supervision of corresponding team members.

• Providing physical therapy services to patients according to the patient plan of care which may include assessing and evaluating therapeutic/rehabilitative/functional status, assessing for muscle strength, mobility, gait, ROM, etc.
• Initiating physical therapy programs and instructing other team members and/or families/caregivers in certain phases of the therapy with which they may work with the patient
• Training patients on the use of prosthetic devices
• Identifying patient and family/caregiver needs for other home health services and referring as necessary
• Preparing and submitting clinical and progress summaries based on patient goals
• Participating in discharge planning of patients
• Completing a 30-day functional reassessment and revising plan and goals accordingly
• Providing physical therapy consultation to home families/caregivers as needed
• Supervising and directing physical therapy assistants (PTA)

Education, Licensure & Certification Requirements

• Possesses a baccalaureate level degree or higher in physical therapy by an accredited organization
• Licensed to practice as a Physical Therapist within the state
• At least two (2) years of experience as a PT.
• Community/home health experience preferred
• Valid Driver’s License and an automobile that is insured and in good working order
