Vista Park Health & Rehab in Douglas, GA is seeking a licensed Physical Therapist to join our compassionate Rehab Services team. You will provide patient-centered therapy, assess and treat individuals, and contribute to outcomes that reflect our mission.

Qualifications

• Must have a Bachelor’s or Master’s degree in Physical Therapy.
• Current, unencumbered Georgia Physical Therapy license.
• Minimum 2 years of experience in a skilled nursing facility rehabilitation setting preferred.

Responsibilities

• Provide treatments meeting patient needs per Rehab Services guidelines.
• Collaborate with Rehabilitation Director and interdisciplinary team.
• Complete clear, accurate documentation for regulatory and payor requirements.
• Supervise and mentor Physical Therapist Assistants.
• Uphold core values and promote teamwork and quality patient care.
