Job summary

PACS is hiring a Staff Physical Therapist to evaluate residents, develop rehabilitation plans, and provide skilled therapy in our post-acute setting. The role emphasizes collaboration with physicians, families, and the health team to support mobility and safety.

The position requires a PT license, a degree in physical therapy, and the ability to document thoroughly while maintaining compliance with regulations.

Qualifications

• Education and/or Experience: Bachelor’s degree in Physical Therapy; Masters/Doctorate preferred.
• Proficient in computer skills for Net Health and Point Click Care.
• Language Skills: ability to read technical procedures; comprehend policies and procedures.
• Mathematical/Reasoning: apply fractions, percentages, ratios, proportions; solve problems.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential per physician orders and policies.
• Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and independence.
• Supervise Physical Therapy Assistants, aides, and students in resident care.
• Communicate progress, goals, and discharge planning with physicians, nursing staff, residents, and families.
• Complete timely, accurate documentation and maintain safe equipment, infection control, and safety protocols.

Education

Bachelor’s degree in Physical Therapy

Masters or Doctorate in Physical Therapy (preferred)
