Job summary

Qualifications

• Graduate of an APTA-accredited physical therapy program with current state licensure.
• CPR certification including BLS/AED training.
• Maintains and updates professional knowledge and participates in ongoing training.

Responsibilities

• Evaluate all patients referred for physical therapy.
• Complete and submit evaluation reports and summaries.
• Communicate evaluative findings and patient needs to referral sources and other departments.
• Ensure therapies align with APTA Ethics and Standards of Practice.
• Provide patient-centered, culturally competent care across all ages.
• Maintain patient treatment schedules and causal episode management.
• Prepare and submit department reports while keeping records confidential.
