Job summary

Escondido Healthcare, LLC seeks an experienced Staff Physical Therapist to evaluate residents, develop treatment plans, and lead therapy services. You will supervise PT assistants, coordinate with physicians and nurses, and participate in care conferences to optimize recovery and discharge planning.

The role requires state licensure, a PT degree (MS/PhD preferred), and proficiency with Net Health and Point Click Care.

Qualifications

• Must hold a valid Physical Therapist license in the state.
• Bachelor’s degree in Physical Therapy; master’s or doctorate preferred.
• Proficient with Net Health and Point Click Care for documentation.
• Excellent written and verbal communication skills.

Responsibilities

• Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Supervise Physical Therapy Assistants in resident care and activities.
• Communicate regularly with physicians, nursing staff, and families regarding progress and discharge plans.
• Participate in Resident Care conferences and Rehabilitation meetings as needed.
• Provide in-services on training programs for staff when required.
• Complete timely, accurate documentation in line with policies and payer requirements.
• Maintain a safe treatment environment following infection control and safety protocols.
• Educate residents and families on goals, home exercise programs, and fall prevention.
• Recommend and facilitate ordering of durable medical equipment to support mobility.
