Job summary

Upperline Health is seeking a Lead Patient Care Coordinator for the Belvedere office. You will support physicians, oversee clinic operations, and lead a team to deliver comprehensive care within a value-based program.

Responsibilities include staffing, KPI monitoring, daily huddles, patient enrollment in Upperline Plus, and higher-level collaboration with regional leadership to ensure patient experience and operational excellence.

Qualifications

• Experience as a Patient Care Coordinator or similar role.
• Podiatry or Orthopedic clinic experience preferred.
• Proficient with medical terminology and computer skills.
• Experience using EMR systems, preferably Athenahealth.
• Excellent communication and interpersonal skills.
• Proven leadership with reliable work history.

Responsibilities

• Oversee daily clinic operations and staff scheduling.
• Lead enrollment and engagement in Upperline Plus care program.
• Ensure clinic KPIs across clinical, operational, and financial metrics.
• Facilitate daily clinic huddles with the team.
• Manage referrals, scheduling cadence for high-risk patients, and chart audits.
• Maintain positive clinic culture and partner with regional leadership.
