Job summary

Upperline Health is seeking a Lead Patient Care Coordinator to oversee the Belvedere clinic, supporting physicians and leading a range of clinical and administrative tasks. The role requires flexible problem solving, collaboration, and a background in podiatry or orthopedics is preferred.

The Lead will manage enrollment for Upperline Plus, staff clinics, monitor KPIs, and ensure top patient experiences. Travel to nearby clinics may be required. Full-time with competitive benefits.

Qualifications

• Previous experience as a Patient Care Coordinator.
• Podiatry or Orthopedic clinic experience preferred.
• Excellent computer skills and strong knowledge of medical terminology.
• Previous experience working in an EMR – athenahealth EMR preferred.
• Effective communicator with excellent grammar and interpersonal skills.
• Demonstrated experience as a motivated and dedicated team leader with a stable work history.
• Reliable transportation and ability to pass background check and drug test.

Responsibilities

• Daily clinic coverage and staffing across clinics with market leadership.
• Accountability for clinic KPIs – clinical, operational, financial – with Regional Manager.
• Facilitate daily clinic huddles with the team.
• Monitor referral conversions and physician schedule templates for high-risk patients.
• Maintain a positive clinic culture and build trust within the team.
• Assist leadership with implementing corporate initiatives in the clinic.
• Perform monthly audits of clinic operations and EMR documentation.
• Be accountable for patient experience as measured by NPS.
• Responsible for clinic and equipment licensure.
• Travel to surrounding clinics to provide support as needed.
• Make timely and high-quality decisions.

About the Lead Patient Care Coordinator Role

Upperline Health is seeking an energetic, compassionate, and driven Lead Patient Care Coordinator who will be responsible for our Belvedere office. The Lead Patient Care Coordinator will assist our physicians in delivering comprehensive health services in the clinic setting and will lead the clinic with a range of clinical and administrative tasks. Candidates must be flexible problem solvers and collaborative team players who are eager to take ownership of patient care and the success of the clinic. Ideal candidates will have podiatry or orthopedic understanding.

Lead Patient Care Coordinator Responsibilities

• All the responsibilities outlined in the patient care coordinator role.
• Deep understanding of Upperline’s comprehensive care program, Upperline Plus, and responsible for patient enrollment and engagement efforts in collaboration with broader care team
• Responsible for daily coverage in clinic and working with other Clinic Leads in market to staff all clinics appropriately.
• Accountable for clinic key performance indicators – clinical, operational, and financial – in collaboration with Regional Manager.
• Facilitate clinic daily huddles with all members of the clinic team each day.
• Responsible for clinic referral conversion, monitoring of physician schedule templates, and adherence to scheduling cadence for high-risk patients.
• Maintain a strong positive culture in the clinic; build trust and respect within the team.
• Assist regional leadership with implementing and maintaining key corporate initiatives and objectives within the clinic setting.
• Perform monthly audits of the clinic operations as well as the EMR documentation.
• Accountable for the clinic patient experience measured by net promoter score.
• Responsible for clinic and equipment licensure.
• Travel to surrounding area clinics to provide support if needed.
• Make timely and high-quality decisions.

Lead Patient Care Coordinator Qualifications

• Previous experience as a Patient Care Coordinator.
• Podiatry or Orthopedic clinic experience preferred.
• Excellent computer skills and strong knowledge of medical terminology
• Previous experience working in an EMR – preferably athenahealth EMR.
• Must be an effective communicator with excellent grammar and interpersonal skills.
• Exceptional customer service orientation featuring an empathetic, compassionate, and professional demeanor with each interaction.

• Reliable transportation and the ability to pass a background check and drug test is required.

Physical Demands and Working Conditions:

• Day-to-day work includes desk computer work and direct patient interaction, facility staff and physicians.
• Position may have extended periods of standing and to lift more than 5 lbs.
• The work environment is characteristic of a health care facility with air temperature control and moderate noise level.

Compensation:

• Compensation is commensurate to compensation for similar positions in the region and based on prior training and experience.
