Job summary

Upperline Health is seeking a Lead Patient Care Coordinator for our Lutherville Timonium office in Maryland. This role leads the clinic and supports physicians with clinical and administrative tasks, delivering high-quality patient care.

The ideal candidate has prior patient care coordination experience, excellent communication, and familiarity with EMR systems. Travel to nearby clinics may be required; a strong team leader mindset is essential.

Qualifications

• Previous experience as a Patient Care Coordinator.
• Podiatry or Orthopedic clinic experience preferred.
• Excellent computer skills and strong knowledge of medical terminology.
• Previous experience working in an EMR - preferably athenahealth EMR.
• Must be an effective communicator with excellent grammar and interpersonal skills.
• Demonstrated experience as a motivated and dedicated team leader with a stable work history.
• Reliable transportation and the ability to pass a background check and drug test is required.

Responsibilities

• All the responsibilities outlined in the patient care coordinator role.
• Deep understanding of Upperline's comprehensive care program, Upperline Plus, and enrollment/engagement with the care team.
• Responsible for daily coverage in clinic and staffing with clinic leads.
• Accountable for clinic KPIs—clinical, operational, and financial—in collaboration with Regional Manager.
• Facilitate daily clinic huddles with the clinic team.
• Monitor scheduling cadence for high-risk patients and ensure adherence.
• Maintain a strong positive culture and build trust within the team.
• Assist leadership with implementing key corporate objectives in the clinic.
