Job summary

Evergreen Treatment Services in Burien, WA, seeks a Screening and Outreach Coordinator to act as the LEAD referrals liaison, delivering screening, outreach, and engagement for individuals involved with LEAD. The role requires coordinating with police, sheriff’s office, and DOC, and providing rapid responses when case managers are unavailable.

The coordinator will perform street outreach, establish strong partnerships with LEAD collaborators, and maintain timely documentation, ensuring referrals

Qualifications

• High school diploma or equivalent required; undergraduate degree preferred.
• Experience with street outreach and trauma-informed care.
• Ability to collaborate with law enforcement and social services to serve mutual clients.
• Understanding of substance use, harm reduction, homelessness, and community resources.

Responsibilities

• Serve as the active point of contact for all LEAD referrals and coordinate with case managers.
• Conduct initial screening and outreach to referred individuals.
• Provide street outreach to engage participants and support intake processes.
• Develop relationships with LEAD partners and other providers to coordinate services.
• Document activities and update program records in the database.
• Attend staff meetings and contribute to program improvements.
