Job summary

PACE Southeast Michigan is seeking a Community Living Associate to support residents in a community setting. The role includes assisting with activities of daily living, meals, and transportation coordination under supervision of the manager and RN/lead CLA.

The CLA will document care, engage with residents’ care plans, and ensure safety and dignity while delivering compassionate, person-centered support in the PACE setting.

Qualifications

• H.S. diploma or GED required.
• Nursing assistant certification or completion of a formal CNA training program.
• CPR certification and immunization as required.

Responsibilities

• Provide personal care up to and including 2-person assist.
• Assist participants with activities, meals, toileting, and mobility.
• Document care provided accurately and on time.
• Coordinate care with the clinical team and family as needed.
• Operate within the established care plan and CMS guidelines.
