Qualifications

• High school diploma or GED required.
• Valid Michigan driver's license.
• Current CPR certification.
• 1 year experience with frail or elderly population preferred.
• Experience in long-term care, hospital, home care, or PACE setting.
• Completion of a basic nursing assistant training program with certificate.
• Mandatory in-service training and screenings annually.
• Ability to work with diverse backgrounds and support independence.

Responsibilities

• Provide personal and supportive care per the care plan.
• Assist with activities of daily living and meal preparation.
• Support transportation to appointments and community activities.
• Document care provided accurately and timely.
• Report deviations or complaints to supervisor/manager.
• Maintain a safe, respectful, and clean environment.
• Collaborate with RNCM/clinical team and lead CLA.
