Job description

Summary

Under the management of the PACE SEMI’s Community Living Manager and the coordination of the Lead CLA, the Community Living Associate (CLA) provides supportive services to participants (residents) in the community setting, during transport, on appointments, or at the day health center while supervised by the manager, registered nurse, or lead CLA. The CLA’s duties are specified in the individual care plan and support residents in achieving maximum self‑reliance and independence.

Key Responsibilities
  • Assist with personal care up to and including 2‑person assistance during 1:1 interaction in the day health center (DHC) and community living (CL) settings.
  • Support activities of daily living, including meal preparation, cooking, feeding, and managing nutritional needs.
  • Assist with toileting and incontinence management.
  • Provide simple treatments as outlined in the care plan.
  • Handle grocery shopping, laundry services, and housekeeping/sanitizing tasks.
  • Coordinate care with participant and DHC per the care plan.
  • Engage residents, address health concerns, and resolve customer‑service issues appropriately.
  • Perform rounds every two hours between 7p.m. and 7a.m.
  • Document care and services accurately and timely; keep manager, lead CLA, and clinical team informed of progress.
  • Report customer‑service complaints and deviations from the schedule to supervisor, manager, or lead CLA.
  • Respond to emergency situations according to established policies and procedures.
  • Work strictly within the established scope of practice.
Qualifications and Knowledge
  • High school diploma or GED required.
  • Current Michigan Driver’s license.
  • Current CPR card required; certification is optional but preferred.
  • At least one year of experience with frail or elderly populations.
  • Experience in long‑term care, hospital, home care, or PACE settings.
  • Completion of a basic nursing assistant training program with certificate of completion from a formal program (Direct Care, Nurse Assistant, Medical Assistant, etc.).
  • Annual mandatory in‑service training (infection control, TB testing, safety training, BLS). Participation in practicum demonstrations and written assessments.
  • Ability to establish and maintain interpersonal and interdepartmental relationships.
  • Respect for the physical, spiritual, and well‑being of participants and co‑workers.
  • Problem‑solving and instruction interpretation skills.
  • Organizational skills and flexible scheduling to coordinate job responsibilities.
  • Sensitivity to individuals of diverse ethnic and cultural backgrounds.
  • Ability to work with participants with dementia and/or developmental disabilities.
  • Capacity to perform duties while frequently interrupted.
  • Meeting standardized competencies (approval by CMS) before independent work; competency monitored through performance evaluations, in‑service training, and staff meetings.
  • Medical clearance for communicable diseases and up‑to‑date immunizations before engaging in direct participant contact.
Working Conditions

The role involves variable conditions with exposure to communicable diseases, chemicals such as cleaning solutions and medications, and the need for frequent walking, bending, and lifting of forty (40) pounds or more. The CLA is supervised regularly by the manager and RNCM in the community setting. Reliable transportation within the PACE SEMI catchment area is required; the CLA must maintain a valid driver’s license, an acceptable driving record, and proof of automobile insurance.
