Job summary

PACE Southeast Michigan is seeking a Community Living Associate (CLA) to provide supportive services to participants in the community setting, during transport and appointments, or in daily health center activities under supervision. The CLA will follow individual care plans and assist with daily living tasks, meals, and coordination of care with the clinical team.

Qualifications

• H.S. diploma or GED required.
• Valid Michigan Driver's license.
• Current CPR card required.
• Certification preferred, but one year of frail/elderly care experience required.
• Experience in long-term care, hospital, home care, or PACE setting.
• Completion of a basic nursing assistant training program with a formal certificate.
• Mandatory in-service training and competency assessments.
• Ability to work with diverse ethnic/cultural backgrounds.
• Ability to transfer/move patients and handle lifting up to 40 pounds.

Responsibilities

• Provide supportive services in community settings, during transport and appointments, or at the day health center under supervision.
• Assist with activities of daily living and meals; 1:1 care as outlined in plan of care.
• Coordinate care with participants and the day health center based on care plans.
• Document care accurately and timely; report deviations to supervisor.
• Respond to emergencies per established policies; maintain scope of practice.
• Participate in in-service training and maintain required competencies.
• Maintain interpersonal relationships and a respectful environment for participants and staff.
• May drive within PACE SEMI catchment area and perform rounds as scheduled (q2 hours after 7pm-7am).
• Adapt to changing acuity levels and participant census; support housekeeping and homemaking tasks as needed.
