Job summary

PACE Southeast Michgan is seeking a Community Living Associate (CLA) to support residents in the community, during transport and appointments, and in the day health center under supervision. The CLA follows each participant’s care plan to promote comfort, safety, and independence.

Qualifications include a high school diploma or GED, a valid Michigan driver’s license, CPR certification, and at least one year of experience with frail or elderly populations; CNA training is preferred.

Qualifications

• H.S. diploma or GED required.
• Valid Michigan driver’s license.
• Current CPR card required.
• CNA training/certification preferred.
• One year of experience with frail or elderly population.

Responsibilities

• Provide personal care up to 1:1 in DHC and CL per care plan.
• Assist with activities and meals.
• Support toileting and incontinence management.
• Coordinate care with participants and the DHC based on care plans.
• Record accurate timely documentation of services.
• Respond to emergencies per policies; maintain scope of practice.
