Job summary

PACE Southeast Michigan is seeking a Community Living Associate (CLA) to provide supportive services to residents in the community setting, during transport, and in the day health center under supervision. The CLA helps residents achieve maximum self-reliance while respecting the program's Ten Principles.

Responsibilities include implementing care plans, assisting with personal care, meals, housekeeping, and coordinating care with the RN and clinical team.

Qualifications

• H.S. diploma or GED required.
• Valid Michigan Driver’s license.
• Current CPR card required.
• Certification is preferred; must have one year experience with frail or elderly population.
• Experience in long-term care, hospital, home care, or PACE setting.
• Completion of a basic nursing assistant training program with certificate.
• Must complete mandatory in-service training and competency assessments.
• Able to work with diverse populations and individuals with dementia.

Responsibilities

• Provide care per participant care plan; assist with daily living activities.
• Personal care up to and including 2-person assist; 1:1 interaction in care areas.
• Assist with meals, toileting, mobility, and homemaking tasks.
• Coordinate care with participant and DHC; document progress accurately.
• Report deviations, attend training, and respond to emergencies as needed.
• Rounds q2 hours after 7pm-7am; drive within PACE SEMI catchment area.
