Job summary

PACE SEMI is seeking a Community Living Associate (CLA) to provide supportive services to residents in a community setting, during transport, appointments, and in the day health center under supervision. The role emphasizes compassionate care, documentation, and adherence to CMS standards.

The CLA must complete required nursing assistant training, hold a valid Michigan driver’s license, and maintain CPR certification. Prior experience in long-term care or PACE environments is preferred.

Qualifications

• H.S. diploma or GED required.
• Valid Michigan Driver's license.
• Current CPR card required.
• Certification is optional though preferred, but must have one year experience with a frail or elderly population.
• Experience in long-term care, hospital, home care, or PACE setting is required.
• Completion of a basic nursing assistant training program and certificate from a formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.).

Responsibilities

• Provide personal care up to and including 2-person assist.
• 1:1 interaction in DHC and CL as outlined in the plan of care.
• Assist with activities and meal preparation / feeding / nutrition needs.
• Assist with toileting and incontinence management.
• Coordinate care with participant and daily health center (DHC).
• Record accurate timely documentation of care and services.
• Report all customer service complaints or schedule deviations to supervisor.
• Respond to emergencies per policies and procedures.
