Community Bridges, Inc. seeks a Lead Navigator in recovery with experience as a Navigator or Peer Support Specialist to help individuals who are homeless. You will conduct outreach, engage clients, coordinate services with RBHA providers, and deliver wraparound supports to end homelessness.

You will mentor Navigator II, guide onboarding, and ensure processes like coordinated entry, housing referrals, leasing up, and discharge are followed by a multidisciplinary team.

Formación

• 1-3 years of health care-related work experience preferred but not required.
• Current AZ driver's license is required.
• Arizona Fingerprint Clearance card, site specific.

Responsabilidades

• Outreach and engagement with individuals who are homeless.
• Coordinate and monitor RBHA and non-RBHA services with wraparound supports.
• Mentor Navigator II to onboard and fulfill program requirements.
• Lead coordinated entry, housing referrals, leasing up and discharge processes.

Educación

Highschool diploma or GED

Associates Degree or higher in Behavioral Health (preferred)
