Job summary

Community Bridges, Inc. (CBI) seeks a Lead Navigator to serve Albuquerque clients experiencing homelessness, providing outreach and wraparound services to help end homelessness. The lead coordinates care with RBHA and non‑RBHA community resources, and supports delivery through a multi‑disciplinary team.

The role requires a recovery background (≥1 year) and at least 90 days of Navigator or Peer Support experience, plus a driver’s license and a clean MVR. Strong mentorship skills are essential.

Qualifications

• Minimum high school diploma or GED; associate degree preferred.
• 1–3 years healthcare experience in behavioral/medical crisis is preferred.
• Or equivalent education/experience combination.
• Valid driver's license and a clear 39‑month MVR.
• Behavioral Health Technician certification per policy.

Responsibilities

• Conduct outreach and engagement with individuals who are homeless.
• Coordinate entry, housing referral, wrap‑around services and discharge.
• Mentor Navigator II and support program onboarding.
• Collaborate with a multi‑disciplinary team to connect care.
