Job summary

Albuquerque Health Care for the Homeless is seeking a Housing Navigator to bridge the gap between crisis and stability in Albuquerque, NM. In this role you will connect individuals in shelters and other temporary housing to lasting housing opportunities and social services, guided by a harm-reduction approach.

Qualifications

• Ability to work with diverse groups of people.
• Excellent verbal and written communication skills.
• Strong interpersonal skills.
• Must have good organizational skills.
• Must have high skill level in attention to detail.
• Comfort with working in variable work locations.
• Valid New Mexico driver’s license with clean driving record.
• Working knowledge of housing voucher programs.
• 1 year of experience in housing or social services with underserved populations.

Responsibilities

• Conduct interviews with clients to determine housing needs
• Administer housing assessments (as needed) for those experiencing homelessness
• Acquire and maintain information regarding available housing resources in Bernalillo County
• Develop and maintain tracking system(s) for funder reporting (in conjunction Housing Manager/Housing Coordinator)
• Document all encounters and contacts made on behalf of client into EHR within 72 hours
• May assist participants in their housing search, including applying for apartments and negotiating with landlords
• Connect clients to other social services in the community as needed
• Complete releases/requests for information, intakes, referrals, and other required documentation
• Work closely with housing voucher, social services, and governmental programs
• Work collaboratively within an integrated team and establish positive relationships with staff
• Perform other duties as assigned
