Community Bridges Inc. is looking for a Client Care Coordinator in Albuquerque, NM, responsible for managing care coordination for individuals and families experiencing homelessness. This role includes coordinating community-based services and completing necessary documentation.

The ideal candidate should have a high school diploma, six months of recovery from substance use or mental health disorders, and must obtain Peer Support or Recovery Coach certification within 90 days of hiring.

Competenze

• Must have a minimum of six months of recovery from substance use and/or mental health disorders.
• Experience in behavioral and medical crisis situations is required.
• Must obtain Certified Peer Support Worker or Recovery Coach certification within 90 days of hire.
